El Meldonium es un inhibidor de la γ-butirobetaína hidroxilasa (BBOX), una enzima clave en la síntesis de la carnitina. Inhibidor de la biosíntesis de L-carnitina dependiente de la GBB hidrolasa. Inhibidor competitivo de la reabsorción renal de carnitina.
