Summary

Subacute granulocytic leukemia (subacute) is a subtype of acute granulomatous leukemia proposed by the Institute of Hematology of the Chinese Academy of Medical Sciences (1962). The clinical features of this subtype are different from those of acute and chronic granulomatous leukemia in that most cases start gradually, with anemia or infection as the first symptom. In a few patients, fever is the first symptom, bleeding is less frequent and mild, petechiae and epistaxis are common, infections are common in boils on the skin, and the liver, spleen, and lymph nodes are usually not enlarged, and if enlarged, the degree is mild.

Principle

Subacute granulocytic leukemia (subacute) is a subtype of acute granulomatous leukemia proposed by the Institute of Hematology of the Chinese Academy of Medical Sciences (1962). The clinical features of this subtype are different from those of acute and chronic granulomatous leukemia in that most cases start gradually, with anemia or infection as the first symptom. Blood picture: in most cases, there is a decrease in whole blood, which is easily misdiagnosed as retrovirus. Hemoglobin and erythrocyte counts are reduced, often more obvious than other types of leukemia, mostly 20-60 g/L. Leukocyte counts are mostly normal or below normal, while a few cases are increased. Classification can be seen in various stages of naïve granulocytes, but the neutrophil granulocytes are predominant. With the progress or deterioration of the disease, most of the patients often have a tendency to increase the white blood cell count, the classification of primary and early juvenile granulocytes also increase accordingly, eosinophils and basophils can be increased, platelets are obviously reduced, and the morphology is abnormal.

Bone marrow image: the bone marrow is mostly hyperplastic or hyperplastic, erythrocyte and megakaryocyte proliferation are reduced, and granulocyte proliferation is obviously active, with neutrophils and neutrophils predominating, and primitive and early juvenile granulocytes are slightly higher than normal. The morphology of neutrophils in this type of leukemia is characterized by an imbalance in the development of the nucleus and cytoplasm, the nuclear chromatin is delicate and loose, the nucleolus is large and obvious, the cytoplasm is abundant, containing a large number of tiny pink, neutral granules, is diffusely distributed, common vacuoles and double-layer cytoplasm, the amount of internal plasma is large, pink, and the external plasma is small in the form of pseudopods, light blue, and Auer's microtubules are rarely seen, with the natural development of progenitor and progenitor cells. Auer's vesicles are rare, and with the natural progression of the disease, the number of progranulocytes and promyelocytes also increases.

Caveat

Do not use chemical stains: peroxidase staining is positive or strongly positive.
