1. Broad specimens

The liver was reduced in size and hardened; the surface was uneven and nodular. The section shows diffusely distributed grayish-yellow round nodules of uniform size (about 0.5 cm or less in diameter). The nodules are separated by thin and narrow intervals of fibrous tissue.

2. Tissue section

(1) The normal structure of the liver has been destroyed and replaced by clusters of hepatocytes surrounded by fibrous intervals, called pseudolobules.

(2) Pseudolobules vary in size and are surrounded by fibrous septa. The number of hepatocytes varies from several hepatocytes to several lobules. The hepatocytes are disorganized and not radial. The central vein was deviated or absent. Hepatocytes have different degrees of lipoatrophy and regeneration (darker hepatocyte plasma, hypertrophied and deeply stained nuclei, sometimes binucleated).

(3) The fibrous septum is narrow and fine, with a moderate amount of lymphocytic infiltration and small bile duct hyperplasia.

Caveat

Diagnostic points: the liver becomes smaller and harder, and the normal hepatic lobular structure is completely destroyed and replaced by pseudolobules
