1. Broad specimens

(1) Ulcerative type: a huge ulcer like crater was seen in the mucosa of the gastric curvature at the pylorus, with a diameter of more than 2.5 cm; the ulcer was irregular in shape, with an uneven base, irregularly elevated margins, and disappeared folds of the gastric mucosa. On section, the structure of the gastric wall layers disappeared and was replaced by grayish-white cancerous tissue, and the muscularis propria was infiltrated by cancerous tissue in the form of cord-like thickening.

(2) Polyp type: There is an irregularly shaped mass on the gastric mucosa surface protruding into the cavity. The surface of the mass is uneven, and in some cases, the center of the mass is necrotic and detached to form an ulcer. The base of the mass is wide, and the grayish-white cancerous tissue is seen on the section, which has infiltrated into the deeper layers of the gastric wall and destroyed the structure of the gastric wall in various layers.

(3) Infiltrative type: the whole gastric wall is diffusely thickened; some of them are as thick as 3~4 cm, with a narrow lumen and disappearance of mucosal folds. Grayish-white cancer tissue is seen to grow infiltratively into the submucosa, muscularis propria and plasma membrane layer, and is completely replaced by cancer tissue. The whole stomach wall becomes hard, like a water-soaked leather sac, which is called limitis plastica stomach.

2. Tissue section

(1) The cancerous tissue consists of glandular ducts of different sizes, shapes and irregular arrangement, with interstitium between the glands.

(2) The cancer cells are columnar or cuboidal, obviously heterogeneous, arranged in monolayers or multilayers, with disturbed polarity, and mucus secretion is seen.

(3) The cancerous tissue grows infiltratively towards the gastric (intestinal) wall, reaching the submucosa and muscularis propria.

Common Problems

Diagnostic Points: The tumor consists of glands of varying sizes and shapes, with infiltrative growth and heterogeneous cells.
